RESUMO

Objective:To evaluate the risk factors of acute kidney injury (AKI) in patients with multiple wasp stings.Methods:Patients with multiple wasp stings were retrospectively enrolled in Hanzhong Central Hospital from September 2010 to November 2020. Based on whether the patients developed AKI, the patients were divided into AKI group and non-AKI group. The general characteristics and laboratory examinations between the two groups were compared. The logistic regression model was used to analyze the risk factors of AKI.Results:A total of 356 patients with multiple wasp stings were recruited in this study, with 196 males (55.1%). The age was 56.0(45.0, 64.0) years old. There were 59 patients (16.6%) with hypertension and 13 patients (3.6%) with diabetes. There were 51 patients (14.3%) in the AKI group and 305 patients (85.7%) in the non-AKI group. Baseline data and biochemical examinations indicated that the two groups showed significant differences in gender, age, sting sites (systemic or local), sting needles, proportions of gross hematuria, leukocyte count, hemoglobin, creatine kinase, alanine transaminase, aspartate aminotransferase, total bilirubin, proportions of urinary protein, and proportions of urine occult blood (all P<0.05). The multivariate logistic regression analysis results showed that the increasing number of sting needles (every 10 needles increase, OR=1.866, 95% CI 1.289-2.071, P=0.001), gross hematuria ( OR=9.770, 95% CI 2.586-36.910, P=0.001), decreasing hemoglobin (every 1 g/L increase, OR=0.016, 95% CI 0.001-0.355, P=0.009), increasing aspartate aminotransferase (every 100 U/L increase, OR=1.311, 95% CI 1.144-1.502, P<0.001), and increasing total bilirubin (every 10 μmol/L increase, OR=1.200, 95% CI 1.008-1.430, P=0.041) were independent influencing factors of AKI. Conclusions:The increasing number of sting needles, gross hematuria, decreasing hemoglobin, increasing aspartate aminotransferase, and increasing total bilirubin are independent risk factors of AKI in patients with multiple wasp stings.

OBJECTIVE@#To review systematically the effectiveness of acupuncture in treatment of chemotherapy-induced bone marrow suppression.@*METHODS@#From the date of database establishment to April 1, 2020, the articles on randomized controlled trials of chemotherapy-induced bone marrow suppression were retrieved by computer from the following databases, i.e. PubMed, Cochrane central register of controlled trials (CENTRAL), EMbase, cumulative index to nursing & allied health literature (CINAHL), JBI database of systematic reviews and implementation reports, CNKI, Wanfang, VIP and SinoMed. Using RevMan5.3, Meta-analysis was conducted. With GRADEpro GDT, the evidence quality was evaluated.@*RESULTS@#A total of 12 articles were included, 10 articles of which were analyzed by quantitative Meta-analysis. Compared with the control group, the improvements in the decrease of post-chemotherapy leukocyte (@*CONCLUSION@#Acupuncture alleviates the decrease of leukocyte, platelet, neutrophile granulocyte and erythrocyte counts and improves the survival quality of patients with chemotherapy-induced bone marrow suppression.

RESUMO

An auxiliary isolation device of cupping therapy for cross-infection prevention is designed to reduce the disinfection steps and be against cross transmission. This device is composed of a disposable isolation unit made of fire proof plastic material and a disposable cup-mouth fixator made of elastic material. The disposable isolation unit includes two parts, the cup neck isolation unit and the inner isolation unit of fire cup. These two parts connect with the disposable cup-mouth fixator. All of those three sections of the device are center-connected ring-like structure. This device can well prevent the direct contact of fire cup with the patient's skin surface, characterized as safety protection, simple operation and saving time and manpower.

RESUMO

The pelvic and acetabular fracture accounts for 3%-8% of all fractures, and is often accompanied with injuries to the bladder, rectum, important nerves and blood vessels. The fatality rate and disability rate are as high as 18%. The treatment of pelvic and acetabular fracture has experienced conservative treatment, surgical treatment and minimally invasive treatment. In recent years, minimally invasive surgery has been widely used in internal fixation of pelvic fracture. The anterior pelvic ring is an important tension bow of the pelvis. For unstable pelvic fracture, the anterior pelvic ring fracture accounts for almost 3/4. The authors review several minimally invasive surgical techniques for anterior pelvic ring fracture, including internal fixation (INFIX), anterior column channel screw internal fixation, pubic symphysis channel screw internal fixation and anterior ring bridge plate osteosynthesis, so as to provide a reference for choice of clinical treatment methods.

RESUMO

Acute traumatic cervical spinal cord injury (TCSCI) is one of the most common severe injuries, and is often accompanied by complications like respiratory infection due to different degrees of paralysis of respiratory muscles, decreased cough function, increased bronchial secretions and bronchial spasm, which seriously affects the respiratory function recovery and enhances mortality rate. The authors review the respiratory management in patients with TCSCI from mechanical ventilation, tracheotomy, oxygen therapy, aerosolized inhalation, assisted sputum extraction, and respiratory function training, in order to provide a reference for clinical nursing work and improve the treatment effect.

Objective To compare the diagnostic results and values among Duke criteria,St Thomas criteria,the modified Duke criteria and domestic criteria for the diagnosis of infective endocarditis (IE) in children.Methods Forty-five children were clinically diagnosed IE,the surgical or the pathologic or autopsy findings were served as the golden criteria for the diagnosis of IE.The children were classified by Duke criteria,St Thomas criteria,the modified Duke criteria and domestic criteria respectively,so as to compare the sensitivity and specificity and accuracy of all criteria for the diagnosis of IE.Results During the forty-five children were clinically diagnosed IE,of whom 32 cases with IE were confirmed by the surgical or the pathologic or autopsy findings; 13 cases were ruled out from the diagnosis of IE.The specificity of Duke criteria,the modified Duke criteria and domestic criteria were all 100%,but the specificity were 92.3% in St Thomas criteria for the 45 cases with clinically diagnosed IE and 26 cases with negative blood culture.For the children with clinically diagnosed IE,the sensitivity and the accuracy were 90.6% and 93.3% in domestic criteria,62.5% and 73.3% in Duke criteria and the modified Duke criteria,87.5% and 88.9% in St Thomas criteria.For the children with negative blood culture,the sensitivity and the accuracy were 76.9% and 88.5% in domestic criteria,7.7% and 53.8% in Duke criteria and the modified Duke criteria,69.2% and 80.8% in St Thomas criteria.The Youden index had no significant difference among domestic standard and Duke standard,Duke modified standard,St Thomas standard for children clinically diagnosed IE (P > 0.05) ; however,the significant difference of the Youden index among domestic standard and Duke standard,Duke modified standard for children with negative blood culture were found(P < 0.01),no significant difference of the Youden index between domestic standard and St Thomas standard was found (P > 0.05).Conclusions With the addition of echocardiographic evidence of endocardial involvement,2 minor criteria as definite diagnostic criteria and some clinical minor criteria,the sensitivity and accuracy of the domestic criteria is superior to those of the Duke criteria,St Thomas criteria and the modified Duke criteria,and the domestic criteria is more suitable for the diagnosis of IE in Chinese children.
